Required Health Treatment Plans

Each School year a new Health Treatment Plan is required by Prince William County Schools for every student with a medical condition which may necessitate treatment at school.

Health Treatment Plans for ASTHMA (Reg 757-5), SEVERE ALLERGIES (Reg 757-2), DIABETES (Reg 757-6),SEIZURES (Reg 757-7) and OTHER PROCEDURES or EMERGENCY health treatments (Reg 757-3)  may be located and printed by clicking on the health condition's regulation number.

To facilitate a positive transition to the new school year, you and your health care provider must complete and sign the corresponding health treatment plan before treatment is implemented at school. Please deliver or fax the completed Health Treatment Plan to Marsteller Middle School between 8 am and 4 pm, Monday through Friday.
